1) Telehealth Consent

Telehealth involves the delivery of healthcare services using electronic communications to connect you with an independent licensed provider.

By using Humanpeptide and submitting an intake questionnaire, you consent to receive healthcare services via telehealth, including clinical review of your intake and, if appropriate, a prescription.

Expected benefits

  • Convenient access to provider evaluation without an in-person visit
  • Timely review of your request for prescription-only treatments
  • Ability to coordinate fulfillment through a pharmacy, if prescribed

Possible risks and limitations

  • Information you provide may be incomplete or inaccurate, which can affect clinical decisions
  • Technical or system issues could cause delays
  • Although safeguards are used, there is a risk of unauthorized access to information transmitted electronically
  • Telehealth may not be appropriate for all conditions, and providers may require in-person care or additional information

Your rights

  • You may withdraw consent to telehealth at any time. If you withdraw, you may not be able to use Humanpeptide services.
  • You may request access to records related to your services, as permitted by law.

2) Independent Provider Relationship

Humanpeptide is a technology platform and does not practice medicine. Medical services are provided by independent licensed providers. Providers use their independent professional judgment to determine whether a prescription is appropriate. No prescription is guaranteed.

3) Health Information Authorization

You authorize Humanpeptide to collect your intake information and share it as needed with:

  • Independent licensed providers for evaluation and prescribing decisions
  • Pharmacies that may fill prescriptions if issued
  • Service providers that support platform operations (e.g., payment processing, security, hosting), as necessary

You agree that you will provide information that is accurate and complete and that you will update information if it changes.

4) Electronic Communications (Email Only)

You consent to receive communications electronically by email regarding your intake, provider review, order status, and required disclosures. (We do not provide live chat/calls and do not send SMS unless explicitly introduced and consented to in the future.)

5) Financial Authorization (Authorization-Only Billing)

By submitting a request and providing payment information, you authorize Humanpeptide (through its payment processor) to place a temporary authorization hold for the amount shown at checkout.

  • We capture payment only if a provider determines a prescription is appropriate and the order is approved for fulfillment.
  • If not prescribed, the authorization is voided/released. Your bank’s timeline for releasing holds varies and is outside our control.
  • If only some requested items are prescribed/approved, denied items may be removed and you will only be charged for approved items.

6) Pharmacy Fulfillment & Shipping Authorization

If prescribed, your medication will be filled by an independent pharmacy and shipped to the address you provide.

You authorize shipment of prescribed medications to your designated address and understand:

  • Delivery timelines are estimates and may vary
  • Once a prescription order is shipped, prescription items are generally not returnable
  • You are responsible for providing an accurate shipping address

7) GLP-1 Subscription Programs (If Enrolled)

If you enroll in a GLP-1 subscription program, you authorize recurring authorizations/charges on the stated cadence, subject to provider approval each cycle.

You may cancel your subscription before the next cycle is submitted for provider review to avoid that cycle’s authorization/capture.

8) No Guaranteed Outcomes

You understand that results vary and no outcomes are guaranteed.

9) Acknowledgement & Consent

By checking the box during intake/checkout, you confirm:

  • You have read and understand this Medical Consent
  • You consent to telehealth evaluation
  • You authorize use and sharing of your information as described
  • You understand prescriptions are issued only if clinically appropriate
  • You agree to authorization-only billing and pharmacy fulfillment terms
